A Step-by-Step Way to Assess Aesthetic Risk

Some anterior implant cases end beautifully and others never quite look right. The difference usually starts before the first incision. Dr. Wazirian walks through each factor he looks at, in the order he uses it.

  1. Why Some Implant Cases Look Better: Patient Expectations

Why the outcome is decided at the consultation, and how to make the patient’s expectations objective by looking at the space, the occlusion and the smile line.

  2. Smoking and the Dynamic Smile Line

Why heavy smokers are high aesthetic risk, and how to assess the smile line while the patient speaks and smiles, not from a static photo.

  3. Gingival Phenotype, Tooth Shape and Black Triangles

Thick vs. thin phenotype, rectangular vs. triangular teeth, and why black triangles (and their cost) are discussed before treatment.

  4. Infection, Papilla Height and Adjacent Restorations

How infection at the site adds tissue loss, the bone level on the adjacent teeth that supports the papilla, and the risk of crowns next door.

  5. The Edentulous Space: Adjacent Implants, Width and Vertical Defects

Two missing teeth vs. one, an implant next to a pontic vs. two implants, and why vertical defects are the hardest to rebuild.

  6. Case Review 1: Replacing a Central and a Lateral

A real case from extraction to final restoration: soft tissue grafting, provisionals and pink porcelain, and what could have changed the outcome.

  7. Case Review 2: A Very High Aesthetic Risk Patient, and Key Takeaways

Failing implants, reconstruction and the prosthetic design chosen, then how to set the right expectations with your patients from the start.
